  4. This mutual release, executed on    [date], between

  9. intended to effect the elimination of any obligations by
  10. either party as hereinafter designated.

  13. parties with respect to that certain contract entered into

  15. which is attached hereto as Exhibit A, both parties have
  16. agreed to settle said disputes and differences by executing

  20. this mutual release, they are relinquishing their respective

  22. and differences, both parties agree that in consideration of
  23. this execution of this mutual release, and for the added
